Findings: Alleged Victim (AV) relies on the facility for his/her care. AV has a history poor balance, decreased mobility, and falls. AV uses a walker and wheelchair for ambulation. AV has had approximately Ten (10) falls in 2023, resulting in AV hitting h/h head, compression fractures, back pain, red mark on the middle of h/h back and a skin tear on h/h right elbow and visible emotional distress related to falls. The facility failed to appropriately care plan and implement reasonable interventions to address AV’s increasing and ongoing falls, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP23-01478 $1500.00 fine assessed
