Findings: The facility failed to implement adequate interventions or appropriately care plan for Witness #1’s (W1) and the Alleged Victim’s (AV) known behaviors. AV and W1 have a documented history of physical altercations with each other on or about June 19, 2023, July 14, 2023, and July 21, 2023, and the only intervention added due to these altercations were to keep them separated and redirected away from each other. On or about August 5, 2023, AV and W1 were involved in another resident-to-resident altercation, in which AV was hitting W1 with a metal water bottle and W1 was punching AV with closed fists. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
