Findings: The Alleged Victim (AV) had a history of falls. AV was bed bound, however, was able to move to the edge of the bed. AV was care planned to have a fall mat at the edge of his/her bed on the floor. On or about August 3, 2023, AV was found on the floor by staff. AV's fall mat was not in place. AV suffered a bruise to his/her hand as a result of the fall. The facility's failure to follow the care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP25-00441 $188.00 fine assessed
