Findings: The facility failed to provide a safe environment and appropriate services according to the Alleged Victim (AV)’s needs, resulting in significant injury after AV fell, and hit h/h on the ground. AV was transported to the hospital and was admitted with a brain bleed. Documentation provided to the department shows On or about August 04, 2023, at approximately 8:00pm, AV was administered a dose of Behavioral Health Medication 75mg and is to be monitored for side effects that include drowsiness, dizziness, and fatigue. Staff were not present with AV when s/he was ambulating in the common area/dining room at the time of h/h fall. The facility failed to provide adequate supervision and care to AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
