Findings: According to documentation the facility failed to follow the Alleged Victim’s (AV) care plan. AV’S care plan states AV is supposed to have h/h catheter drained at least three times per day. On or about July 25, 2023, AV was treated at the hospital for catheter issues. AV experienced pain and unreasonable discomfort. The facility failed to ensure care plans were followed, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
