Findings: The facility failed to implement interventions and appropriately care plan related to Witness 1's known behaviors and recent increase of altercations. The failure resulted in multiple physical altercations with staff and other residents, including the incident where s/he kicked the Alleged Victim’s (AV) three times, causing unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
