Findings: According to interviews and documentation provided to the department staff know to use the chair with the back on it when assisting Alleged Victim (AV) with a shower. On or about August 04, 2023, AV was being assisted with a shower and using a chair without a back, resulting in AV sliding off the shower chair and sustaining a skin tear to h/h elbow. The facility failed to provide a safe environment, and care plan appropriately around AV’s needs, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP23-00732 $188.00 fine assessed
