Findings: Alleged Victim (AV) and Witness 1 (W1) have a known history of verbal aggression towards one another. Staff are required to keep AV out of sight of W1, in effort to keep the residents apart. On or about August 9, 2023, W1 walked alone through the dining are room and once outside, began to hit AV multiple times in the head. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP24-00197 $250.00 fine assessed
