Findings: On or about August 13. 2023, Alleged Perpetrator #2 (AP2) was assisting the Alleged Victim (AV) with a brief change. AP2 grabbed AV's arm roughly during the brief change, causing AV to sustain three skin tears on his/her arm. AP2's actions are a violation of resident rights, are considered neglect of care and constitute physical abuse. The facility's failure is a violation of Oregon Administrative Rules.
