Findings: Witness 1 (W1) has a known history of conflicts with resident and staff. On or about August 16, 2023, W1 was seen pushing his/her walker at Alleged Victim’s (AV’s) legs. When AV was done smoking, he/she hit his/her pendant for assistance to get by W1’s walker as W1 was ignoring AV and not moving aside. The facility failed to provide a safe environment for AV due to W1’s known history of conflict, putting AV in risk of harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP24-00124 $188.00 fine assessed
