Findings: The Alleged Victim (AV) is care planned as a two person transfer. AV tends to hold onto the wheelchair during transfers and while doing this, it can cause scratches to AV. Alleged Perpetrator #2 (AP2) and Alleged Perpetrator #3 (AP3) were re-positioning AV in his/her wheelchair, and after the action, AV was found to have a skin tear on his/her arm. The facility's failure to outline how AV hangs onto the wheelchair, and how staff should respond to that wasn't properly outlined in the care plan. The facility's failure to ensure clear directions to staff while providing care is a violation of resident rights, is considered neglect of care and constitutes abuse.
