Findings: The Alleged Victim (AV) is care planned as a two person transfer. On or about August 14, 2023, it was discovered that AV had a bruise on his/her right chest. On or about August 20, 2023, AV was sent to the hospital and was diagnosed with a fractured rib. During the course of the investigation, and based on information from AV, it was determined that AV was transferred by Alleged Perpetrator #2 (AP2) without assistance on or about August 12, 2023, causing AV's fractured rib. Alleged Perpetrator #2 (AP2) failed to follow AV's care plan of using two people to transfer AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility's failure to ensure the care plan was followed is a violation of resident rights, is considered neglect of care and constitutes abuse.
