Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about March 1, 2023, AV exited the secure memory care unit through the stairwell exit. The exit door AV exited was not locked, the door required a key to lock and staff did not know the location of the key. The facility failed to provide a safe environment for AV, which put AV in risk of harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP24-01105 $500.00 fine assessed
