Findings: The facility failed to follow the Alleged Victim’s (AV) care plan. AV’S care plan states AV needs to be assisted with toileting and brief change. On or about August 25, 2023, AV was found with soiled briefs. The failure resulted in AV experiencing unreasonable discomfort and a loss of dignity, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
