Findings: According to the documentation, the facility failed to follow the Alleged Victim’s (AV) service plan by ensuring the chair alarm was on and functioning. The failure resulted in the AV falling out of their chair and fracturing their neck requiring evaluation at the hospital and causing continuous pain, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
