Findings: The Alleged Victim (AV) is a known fall risk and has fallen from bed previously. AV had been care planned to have his/her call pendant within reach and have a rolled up blanket on the edge of the bed to assist with AV not falling from the bed. On or about August 26, 2023, AV was found on the floor after falling from the bed. AV suffered redness to his/her hip and was on the floor for an unknown amount of time. The blanket was on the opposite side of the bed by the wall and AV's call pendant was at the foot of his/her bed. The facility failed to follow AV's care plan, which is a violation of resident rights, is considered neglect of care and constitute abuse.
