Findings: Both the Alleged Victim (AV) and Witness 1 (W1) rely on the facility for care. W1 has a history of aggressive behaviors resulting in resident-to-resident altercations. According to an investigation, on or about August 26, 2023, W1 hit AV in the ribcage with a walker, causing unreasonable discomfort. The facility failed to implement effective interventions to address W1 aggressive behaviors and provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
