Findings: On or about August 27, 2023, the Alleged Victim (AV) and Witness #1 (W1) engaged in an altercation where W1 pushed AV and knocked him/her to the ground. After the incident, AV and W1 were put on alert for staff to monitor behaviors and intervene if necessary. Later that same day, AV and W1 engaged again, where W1 pushed AV to the ground, resulting in AV having a swollen finger. The facility's failure to ensure a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
