Findings: Alleged Victim (AV) is known to be a high fall risk per AV’s care plan, but no interventions or directions are listed. On or about August 24, 2023, AV was found in front of the building on the ground. AV had two prior falls, one on May 26, 2023, and the other on July 10, 22023. As a result of the most recent fall, AV sustained an injury. The facility failed to properly plan care around AV’s known history of falls which puts AV in risk of harm and is a violation of resident rights, is neglect of care and constitutes abuse.
