- Findings
- On or about August 9, 2023, Alleged Perpetrator 2 (AP2) physically abused Alleged Victim (AV) while assisting AV with his/her Activities of Daily Living. Based on investigation and facility documentation, AP2 was rough with AV, pushing AV back into bed when AV kept trying to get out of bed, causing AV to knock his/her knees against the wall causing pain and unreasonable discomfort. AP2's action is considered physical abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
