Findings: Alleged Victim (AV) is a 2-person transfer with a Hoyer lift. On or about August 6, 2023, AV was found face down on the floor up against the kitchen counter with blood on the carpet near AV’s feet. AV’s sling net was left dangling underneath AV getting caught with the power chair wheel. The facility did not have the proper Hoyer for AV and was being transferred with another resident’s Hoyer. As a result of the fall, AV received a laceration to their right foot which requires stiches, contusion of their right shoulder, and bruising to their right eye. The facility failed to follow AV’s care plan but no using a proper Hoyer for AV, which is a violation of resident’s rights, is neglect of care and constitutes abuse.
Sanction: ALFCP24-00127 $750.00 fine assessed
