Findings: The Alleged Victim (AV) relies on the facility to provide care and services. According to an investigation, the AV was restrained by the facility staff on multiple occasions which caused AV physical and emotional distress. This was witnessed by multiple facility staff and they failed to report the abuse. The facility failed to provide uphold the state's mandate on mandatory reporting of abuse, resulting in physical and emotional harm to AV which is a violation of resident rights and constitutes abuse.
