Findings: The Alleged Victim (AV) had a history of falls, however, AV's most recent service plan update did not indicate his/her fall record and there was no specific interventions to prevent AV's falls. AV suffered an unwitnessed fall on August 20, 2023 and another fall August 25, 2023. The facility failed to properly care plan for AV's falls, placing AV at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
