Findings: Alleged Victim (AV) relies on the facility for a safe environment. Witness 1 (W1) and AV are roommates. AV and W1 have history of arguing. AV frequently complains to staff about W1 being mean to AV, and not wanting to live with W1. As a result, to W1 treatment cries, self-isolate and will withdraw from activities that s/he enjoys avoiding W1. The facility failed to address AV concerns and the facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
