Findings: Alleged Victim (AV) had a history of falling when ambulating to the restroom that was known to the facility prior to September 4, 2023. AV’s care plan did not include any interventions related to preventing falls when ambulating. On or about September 4, 2023, AV was found on the floor after s/he had gotten up to go to the bathroom and in the process lost h/h balance and fell. AV suffered a head injury and a skin tear to h/h elbow. The facility failed to appropriately care plan and implement reasonable person-centered fall interventions to address AV’s prior falls, which is violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP24-00374 $500.00 fine assessed
