- The Alleged Victim (AV) was in a Nursing facility and returned to the facility on August 24. 2023. AV was to receive bowel medication. AV did not receive this medication from August 24, 2023 to August 29, 2023, placing AV at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
