Findings: The Alleged Victim (AV) was a known fall risk with a history of falls. According to an investigation, on or about August 23, 2023, AV experienced an unwitnessed fall from their bed, resulting in bruising to upper and lower extremities. The facility failed to provide appropriate care planning and interventions to mitigate the risk of injury due to AV's falls, which is a violation of resident rights, is neglect of care, and constitutes abuse.
