- Findings
- The facility failed to monitor and provide appropriate supervision related to the Alleged Victim’s (AV) known history of falls. On or about August 13, 2023, AV experienced a fall that resulted in a carpet burn on his/her right knee and bleeding from AV's upper lip and right eyebrow which caused unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
