Findings: According to the documentation, the facility failed to provide a safe environment for the Alleged Victim (AV) to ensure they could not elope from the secured facility. On or about September 09, 2023, the AV was able to leave the locked facility without staff knowing they left. The AV left the facility and was brought back to the front desk by a family member of another resident. Upon inspection of the back door, the facility found it would not close without additional force, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
