Findings: Alleged Victim (AV) relies on the facility for his/her care. AV has a history of falls and requires staff assistance with transfers and toileting. On or about September 01, 2023, AV was evaluated in the emergency room for head pain and bruising found on h/h back. Injuries are related to two unwitnessed falls occurring on or about August 26, 2023, and August 28, 2023. The facility failed to appropriately care plan and implement reasonable interventions to address AV’s ongoing falls, which is violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP23-00706 $500.00 fine assessed
