Findings: Witness #1 (W1) is known to have inappropriate behaviors with the Alleged Victim (AV), however, W1's care plan was not updated to address how staff are to prevent W1 from negatively interacting with AV. The facility failed to protect AV from W1, which lead to AV being fearful of W1. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
