Findings: Witness #1 (W1) is known to have inappropriate behaviors with the Alleged Victim (AV), however, W1's care plan did not adequately address ongoing issues to keep W1 away from other residents. The facility failed to properly care plan, which caused AV emotional harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
