Findings: Witness 3 (W3) has a history of aggressive behaviors towards staff and other residents who bother W3. On or about September 5, 2023, while Alleged Victim (AV) was trying to get up from the couch, W3 elbowed AV. During the incident Witness 2 (W2) approached W3 two times to hit W3 with the back of his/her hand trying to protect AV from W3. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP24-00076 $500.00 fine assessed
