Findings: On or about September 9, 2023, the facility failed to implement interventions or follow Witness #1’s (W1) care plan which states W1 needs 1:1 supervision due to behaviors. The failure resulted in W1 kicking the Alleged Victim (AV) and causing a bruise. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
