Findings: • Alleged Victim (AV) relies on the facility for h/h care. On or about September 11, 2023, it was discovered AV was not provided incontinence care overnight, leaving AV and his/her bed soaking wet from urine. AV experienced itching, odor, and serious loss of personal dignity. The Alleged Perpetrator (AP2) had previously been formally reprimanded on two occasions for not providing care to residents, without improvement. AP2 and the facility failed to provide care is a violation of resident rights, is neglect of care and constitutes abuse.
