Findings: The Alleged Victim (AV) was a known fall risk with a history of falls. The facility failed to provide appropriate services according to the Alleged Victim (AV)'s needs, relating to his/her change of condition, number of falls with lack of appropriate interventions. According to an investigation, AV experienced multiple unwitnessed falls between August 8, 2023, and September 12, 2023, resulting in pain and head injuries. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
