Findings: The facility failed to assess and intervene Alleged Victim's (AV) change of condition. Based on facility documentation and interviews, AV had two documented injury falls on or about September 8, 2023, and facility failed to provide appropriate health assessment, oversight and monitoring in a timely manner. AV was sent to the hospital on or about September 11, 2023, and was diagnosed with a UTI and fractured pelvis. The facility's failure caused AV unreasonable discomfort which is a violation of resident rights, is considered neglect of care and constitutes abuse.
