Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about September 14, 2023, at about 8:30am, Witness 1 (W1) went to AV’s room and knocked on his/her door, AV opened the door and W1 attacked AV. As a result of the altercation W1 ended up cutting AV’s wrist and then chocking AV with his/her hands around AV’s neck. Staff found W1 in AV’s room with his/her hands around AV’s neck chocking him/her. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP24-00180 $1500.00 fine assessed
