Findings: Alleged Victim (AV) as a known history of wondering into other resident’s rooms on a daily basis. On or about September 10, 2023, AV wondered into Witness 1’s (W1’s) room. AV’s family said they witnessed AV walk into W1’s room. While in W1’s room, AV stated W1 was pulling AV and grabbing AV. As a result of the altercation, AV sustained a fall. AV has been involved in four (4) other altercations due to his/her wandering. AV’s service plan states he/she is to be escorted by staff to and from all meals to prevent altercations, however, staff do not always escort AV to and from meals, putting AV in risk of harm. The facility failed to follow AV’s service plan, which is a violation of resident rights, is neglect of care and constitutes abuse.
