Findings: The Alleged Victim (AV) is care planned for two person assist for most cares, including showers. On or about September 17, 2023, Alleged Perpetrator #2 (AP2) was showering AV by him/herself. AP2 showered AV in his/her wheelchair, and while attempting to remove AV from the shower, the wheels got caught on the shower lip and AV fell forward from his/her chair, causing a laceration to his/her scalp. AV was sent to the hospital where he/she received 13 stitches to his/her forehead. AP2 failed to follow the care plan to have a second person assist with showering AV. AP2's actions are a violation of resident rights, are considered neglect of care and constitute abuse. The facility's failure to follow the care plan and ensure enough staff are available to assist is a violation of resident rights, is considered neglect of care and constitutes abuse.
