Findings: The Alleged Victim (AV) is a known fall risk and has suffered multiple falls. On or about September 17, 2023, AV was found on the floor of his/her apartment, after trying to use the restroom without assistance. No new interventions were put into place after AV's prior fall on September 14, 2023. The facility's failure to properly care plan for falls is a violation of resident rights, is considered neglect of care and constitutes abuse.
