Findings: Alleged Victim (AV) had a known history of wandering into other residents’ rooms. Witness 1 (W1) has a history of being protective of his/her things and not wanting others in his/her room. On or about September 18, 2023, AV went into W1’s room, resulting in W1 punching AV in the back. The facility failed to effectively care plan and/or implement interventions to keep AV safe, which is a violation of resident rights, is neglect of care and constitutes abuse.
