Findings: The Alleged Victim (AV) suffered a significant change of condition on or about September 14, 2023, due to a weight loss and decline in activities of daily living. On or about September 21, 2023, AV returned from the hospital after an injury fall. On or about September 23, 2023, ti's noted that AV is continuing to use the restroom by him/herself, placing him/her at risk for harm. The facility's failure to properly care plan for AV's safety from falls is a violation of resident rights, is considered neglect of care and constitutes abuse.
