Findings: On or about September 21, 2023, the Alleged Victim (AV) was found in his/her room with a laceration to his/her forehead, due to a fall in his/her bathroom. AV was sent out to the hospital for treatment. The facility's failure to provide a safe environment for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
