Findings: The facility failed to implement interventions, appropriately care plan, and provide appropriate supervision to Witness 1 (W1) according to his/her known behaviors and prior altercations. The failure resulted in an unwitnessed physical altercation with the Alleged Victim (AV) causing AV to fall and sustain a fracture, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
