Findings: The Alleged Victim (AV) is a known fall risk and has suffered multiple falls from June 2023 to September 2023. AV was care planned for safety checks every two to three hours, however, due to limited staffing, the safety checks were not regularly completed. The facility's failure to follow the care plan placed AV at risk for serious harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
