- Findings
- The Alleged Victim (AV) suffered 17 non injury falls between July 22, 2023 and September 11, 2023. AV was not injured in the falls, however, the number of falls placed AV at risk for harm. The facility failed to implement meaningful interventions to alleviate the number of falls. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
