Findings: The Alleged Victim (AV) is known to exit seek and leave the facility. AV is to be re-directed back to the facility if seen out on campus by the facility gates, waiting for cars to come in and out and AV will leave while the gate is open. On or about September 24, 2023, staff could not find AV for approximately an hour. AV was found trying to cross a busy 4 lane highway by a motorist during rush hour traffic. The motorist took AV to the local police department, as AV didn't know his/her address and AV was returned to the facility by police. The facility's failure to provide a safe environment placed AV at risk for serious harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
