Findings: Between June 8, 2023 and October 1, 2023, the Alleged Victim (AV) suffered approximately 13 falls, resulting in redness, skin tears, bruising to his/her back, swelling to his/her forehead and pain in his/her elbow. Interventions listed during this time were offer assistance, frequent checks, keep AV's walker near, and to have clear pathways. AV continued to have falls with these interventions in place. No new interventions were put into place to assist with keeping AV safe from falls. The facility's failure to properly care plan to ensure AV's safety is a violation of resident rights, is considered neglect of care and constitutes abuse.
