Findings: The facility failed to provide appropriate services and care plan according to the Alleged Victim’s (AV) needs. According to an investigation, on or about September 29, 2023, AV was not provided breakfast prior to facility staff giving AV their insulin to administer, which caused AV to experience low blood sugar and fall. AV was transferred to hospital for treatment. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
